Problem: The repository changed after the checkpoint was recorded.
Evidence: Saved HEAD differs from the observed HEAD.
Cause: Work continued in the repository without a new checkpoint being submitted.
Required fix: Create a new implementation checkpoint, or reconcile the repository change.
Who does it: Claude Code, or an authorised developer.
Verified by: Repository state matches the new checkpoint.
Resulting state: Resume-safety projection recalculates.
